The course we gave at the 2025 IEEE Electrical Insulation Conference. The course starts with an overview of the RCFA process emphasizing the simple 5-why and complex analysis using PROACT. We will cover tools and resources for forensic investigations including pre-failure histories such as vibration, infrared, insulation resistance, voltage, current, electrical signature analysis, partial discharge and more. The use of additional investigative tools such as microscope, borescope, and other visual and investigative methods. We will have several walk-through examples such as wind turbine wedge and insulation failures, induction machines at low and high voltage and inverter machines. We will also discuss equipment dissection. Apply critical thinking to machinery failures; Recognize common causes of system failures; Understanding operating context.
